Rapport du système de surveillance FoodNet pour l'année 1997 : États-Unis
Le CDC a publié sa "Surveillance Summary of FoodNet" (les Données sommaires sur la surveillance de FoodNet) de 1997, un réseau national de surveillance active qui détermine la fréquence et la sévérité des intoxications d'origine alimentaire ainsi que la proportion des intoxications alimentaires provenant de certains aliments et décrit l'épidémiologie des nouveaux pathogènes d'origine alimentaire bactériens, parasitaires, et viraux. En 1997, FoodNet a effectué le suivi de neuf pathogènes : la campylobactérie, l'escherichia coli O157:H7, la listeria, la salmonelle, la shigella, le vibrio, le choléra, le cryptosporidium et la cyclospora. La campylobactérie est le pathogène le plus souvent diagnostiqué tandis que les infections dues au pathogène listeria ont causé le taux le plus élevé d'hospitalisation et presque la moitié des morts rapportées. FoodNet a démontré que le bœuf haché pas assez cuit était la première source d'origine alimentaire des infections dues au pathogène e. coli O157:H7.
Contrairement aux résultats obtenus lors d'enquêtes antérieures, les hamburgers mangés dans des établissements de restauration rapide n'étaient pas associés à l'infection, ce qui laisse supposer que des modifications récentes apportées à cette industrie pourraient avoir réduit la proportion d'infections dues au pathogène e. coli O157:H7 provenant de cette source.

Le comptage des TIAC aux USA est différent de celui
pratiqué en Europe, en France notamment.
Le Center for
Disease Control d'Atlanta a par exemple l'habitude d'appliquer un
coeffiscient multiplicateur "de coorection" au nombre de
cas déclarés pour tenir compte des non déclarations
et défauts de diagnostic, afin d'estimer le nombre de cas
réels. Ces facteurs de correction varient de 2 (listériose,
botulisme) à 38 (diarrhées non hémorragiques).
Les
chiffres reportés précédemment sont
difficilement interprétables si on ne sait pas comment ils ont
été collationnés et établis.

L'article cité ci-dessous est une tentative d'estimer le
nombre de maladies et de décès dus à des
bactéries, virus et parasites transmis par les aliments aux
Etats-Unis. Il utilise des coefficients de corrections appliqués
aux chiffres de maladies déclarées ou estimés à
partir d'enquêtes ponctuelles. Etes-vous certain que les
chiffres cités dans le message de G. Barma ne sont pas les
données brutes, avant application des coefficients de
correction ?

The numbers for infectious diseases is taken
directly off of the trend tables provided by the Centers for Disease
Control (CDC) in Atlanta. Here is what the CDC says about the
numbers:
"Cases reported by state health departments to
CDC for weekly publication are provisional because of ongoing
revision of information and delayed reporting. Case counts in these
query tables are presented as they were published in the MMWR .
Therefore, numbers listed in later MMWR weeks may reflect changes
made to these counts as additional information becomes
available."
What you are looking at are only reported
cases, not estimations of undeclared cases. The CDC has always
said undeclared cases would significantly raise the numbers of
reported incidents.

Even though the United States has one of the safest food supplies in
the world, foodborne disease poses a continual, significant threat to
public health. The Centers for Disease Control and Prevention
estimate that each year foodborne illness cause 76 million
sicknesses, 325,000 hospitalizations and 5,000 deaths. Perhaps most
alarming, 82% of foodborne illnesses have unknown origins.
As the
food supply and distribution system continues to become increasingly
global, monitoring the safety of the food supply has greatly
increased in complexity. It is also important to recognize that
substantial effort is required because of the emergence of new
strains of foodborne pathogens and because of the increasing consumer
demand for products with few, if any, preservatives and for fresh,
minimally processed products. Thus the development and adoption of
current and new technologies to reduce foodborne illness is critical
to maintain current food safety as well as provide opportunities to
further enhance the safety of the food supply.
To advance the
adoption of technologies, leading food safety experts from industry,
government, and the research community came together for a work
conference to identify and remove barriers to the adoption of food
safety technologies. Their discussions, case studies, and findings
have been documented in a report available from the National
Technical Information Service.
