Studies of the enteric flora of farmers have been carried out
by Nijsten et al. (1994) who determined antibiotic resistance of E.
coli from faecal samples of pig farmers, abattoir workers and
suburban residents. Pig farmers showed the highest percentages of
resistance and suburban residents the lowest. The highest rates of
resistance in the abattoir workers and the urban residents group were
for oxytetracycline and amoxycillin respectively, and in the pig
farmers for sulfamethoxazole. No correlation could be made between
recent antibiotic use or recent hospital stay and the prevalence or
degree of resistance in any of the three groups. However, 5% of pig
farmers and 8% of abattoir workers had used antibiotics during the
previous three months whereas none of the urban residents had done
so. High levels and prevalence of resistance to neomycin were
observed in pig farmers. Neomycin is used frequently in pigs but
rarely in human medicine.
Nijsten et al. (1996a and b) further
investigated the links between the E. coli from farmers and their
pigs. Antibiotic resistance patterns in 266 E. coli from faecal
samples of pig farmers and 285 samples from their pigs were compared.
E. coli from pigs were significantly more resistant to a range of
antibiotics, and were usually resistant to oxytetracycline and
streptomycin, or oxytetracycline, streptomycin and sulfamethoxazole.
The corresponding farmer isolates were mainly resistant to a single
agent such as amoxycillin or sulfamethoxazole. Only 4% of the farmer
E. coli and those of pigs from the same farm shared resistance to the
same antibiotics.
The results suggested that the resistance of
the faecal E. coli of farmers and their pigs was distinctly
different: 74% of the pig isolates and 43% of the farmer isolates
were resistant to two or more antibiotics, whereas 15% of the pig
isolates and 34% of the farmer isolates were fully susceptible. In a
development of this study, ten farmers were found to have the same
resistance patterns as their pigs and 13 to have different
antimicrobial resistance patterns (Nijsten et al.,1996b).The
differences in antibiotic resistance patterns observed between farmer
and pig strains suggested that pigs are not the most important source
of resistant bacteria in farmers. The fact that pig farmers harboured
more resistant bacteria than the normal population, shown in a
previous study (Nijsten et al., 1994), may have been a result of
direct contact with antibiotics in medicated pig feed rather from
direct contact with the pigs.
